Sally, formerly 42-year-old Samuel, listened to what the doctor said.  He was now a little girl, no older than ten.  

The doctor explained what Samuel must do to fit into his new life.  Samuel expected that it would be a lecture on hygiene, but instead, it was how to play like a girl.  He was expected to spend time playing with dolls, drawing in coloring books, watching cartoons, and other girlish pleasures.  Sally didn't want to do any of it, but he had little choice in the matter.  He was a girl now and would stay that way.
