I read Elisabeth Sladen's autobiography (Sarah Jane Smith), and she almost died during the filming of this story arc - she was on an out of control hoverboard type thing racing across the underground river, and almost got swept away but the main stuntman dived in and saved her. In fact, they had a lot of dodgy stuff happen during the shooting, which they attributed to The Witch of Wookie Hole not wanting them there. I always think of that when I rewatch these episodes.
