Survey for Reports / Helping out with Investigation of potential Mod Conflicts & Issues (Patreon)

Hello everyone!
I am almost done with a little update for WHW module 8 which will smooth out some things and add a tiny feature or two, but what remains more important is this: 

You all know by now that some of you are having some things not work in the module 8 of WooHoo Wellness mod. Some of this has been linked to other mods conflicting with the module by users, yet I have come to the conclusion that only a broad survey could help truly figure out some of it, so here comes... a google form survey. 

It would be helpful if you answered it no matter if you are having the issues or not. I need answers from both situations to be able to do anything with it. The more people participate, the better chances are at things being clearly identified.

Here goes:

(fully anonymous, does not require to log in and does not record anything but your answers)

The hypothesis in comments here has been that Wonderful or Wicked Whims prevent Sims from properly getting pregnant as surrogates - your answers on the survey will help to see if it is or not the case. The other issues remain obscure and mysterious and may not be due to conflicts or may be.

Keep in mind I haven't been able to test every possible mod combination for reasons of a personal nature as well as all the usual practical reasons.
