There was an alternate version of the Kara/Lena scene at the wedding filmed where they do kiss. I have no idea what they wanted to do with that version, except maybe show it as fan service. It can be found on YouTube.

The audience knew that Cat figured out Kara's identity. Kara had flown off when she acknowledged it, though. Why was their mom showing up random? Alex's daughter was kidnapped. I would think all family would show up for that. I don't think Kara coming out screws Superman. The Kent's and the Danver's have no relation. Only Cat knew that they were cousins (and that was pre-Crisis). It would make Alex and her family more vulnerable, but they would be anyways. James was already out as Guardian and Kelly is his sister. They would already be a target for someone wanting to hurt Guardian or Supergirl.
