I've gone into mortimer tower and I don't see any witch near there. Where is this witch supposed to be? Her exact line is "Thanks to your efforts, we've dealt with the kidnappers. Now we should focus on this nasty cult."

Also, Ayane says "Be careful of that witch" but I've seen no witch since rescuing Serena and I can't go back there now

Yeah, in the previous dialogue with Serena, she mentions that the witch wanted to use Mortimer’s tower. The only way up is blocked - inspect the rubble up from the crystal cave teleport and Ada will be able to notice a trace from teleportation spell. Use the new teleport to get higher into the tower.
