The end I always took it as he didn't regress, (thats why he doesn't answer/turn around when Mark Ruffalo calls out to him as Teddy) he just chose to 'die as a good man', or even just believing he is in his final moments.

at the end he didnt fall back in hes madness , he decided to accept it and die ''as a good man''in my opinion , but its a great open ending because you can see it eiter way.great movie
