Smallpox has a crazy history. From what I remember, it was endemic across Europe at least, responsible for a high infant mortality rate. That in turn meant a) most adults were survivors with immunity b) being pox-faced was a common thing Smallpox famously devastated native tribes because they had no immunity. The smallpox vaccine was massively effective, but there was a community (in India I believe) that refused. In order to wipe out the disease worldwide, these people were forcibly vaccinated. Brutal, but successful.
