I think you misunderstood a little bit of what not-geto was talking about. Understandable, as a lot of the stuff in JJK is really hard to make sense of, especially on a first time watch. Basically, what not-geto has done is make pacts with sorcerers from up to around 1000 years ago where he turned them into cursed objects after death, much like Sukuna did. He then fed these cursed objects to assorted regular people and some of them fell into a coma like Tsumiki did. However, the objects were still sealed and so they didn't incarnate like Sukuna did as soon as Yuji swallowed the finger. But, not-geto released the seal on them causing the sorcerers from the past to incarnate in their hosts. He used idle transfiguration to strengthen the people so they could become hosts without dying. He also transformed many modern people who have techniques but didn't have the ability to use them so that they now can. Also, when not-geto was saying that "it's starting again" he meant that the Heian period was returning. This period was mentioned in the 2nd episode of S1 as the golden age of jujutsu where sorcerers were far stronger than they are now and is the time that Sukuna is originally from. Btw, JJK manga fans are the worst I have ever seen when it comes to spoilers so be careful. I had the 2 biggest moments in the entire series spoiled for me by youtube shorts titles and thumbnails in my recommended section. I wanted to stay as an anime only but I was forced into reading the manga after S2 ended because I just couldn't stand being spoiled so much.
