Earlier this year we streamlined our crediting process by moving to Google Forms. We can cross-reference these responses with our patron data to make sure everyone is verified and our most generous donors are credited first.

Please submit this form if either of the following apply to you:

• You are currently in the credits but have not filled out this form (or don't remember doing so).
• You are not currently in the credits but want to be in the credits.
