In this episode, before Rem is devoured by the Sin of Gluttony, the brief flash of an image actually shows an adult Rem holding her child with Subaru. If you pause at the right moment, you can see it. There is a "Re:Zero" route featuring a "What if" story where Rem agrees to run with Subaru after his confession. The two marry, have children, and live together. Some images from this alternate future are shown right before Rem's tragic fate, but they pass so quickly that most viewers miss them

To answer the question about the knife that keeps getting shown by Rem's bed, it is the shows very subtle hinting at the fact that Subaru is suicidal. It is spelled out much more overtly in the books. But it deson't really come up in the show. So it was kind of a throw away detail but kinda not for the folks who already knew it.
