When Dean tells Cas "I need you," that really pulled Cas back from the ledge. It was one thing Naomi didn't anticipate in all the scenarios she had Castiel play out while she was "fixing" him. Fun fact, in the script, Dean was supposed to say "I love you," but Jensen chose to change it to "I need you." As much as I would have loved to hear Dean tell Cas he loves him, Jensen made the right call here. It would be out-of-character for Dean to openly express his feelings like that.
