The reason why Luffy can use gears and no one else, is because he is Rubber. It got pretty much explained by Lucci in Enies Lobby. Gear 2 Luffy uses the Rubber to Pump his Blood which raises his heart rate, so basically he gets adrenaline. But if a normal person would/could do that, their heart would probably explode at some point. But since Luffy is rubber his Heart can endure it more. And well Gear 3, Luffy pretty much blows Air into his Body, which also only works because he is rubber.
