14 minute mark of your video the hero guy says that he was reborn in a differant world. So he was born in a unknown world, was summoned to this world as the hero, died it seems in that battle against that tomo demon lord person. Was then reborn in the world with his new childhood friend (grew up again it seems from the end thingy) and now was summoned back to this world 5 years later in this worlds timeline in his new body (hence why he looked differant). I'm anime only so might be wrong but thats what I gathered from what he says in the episode.

No worries easy to miss a random bit of dialogue. I liked how the childhood friend told him back in the school in her world that he needed a haircut and she would cut it for him and thats the last thing she does before they part ways.

Both "Yuki" and "Setsu" can be written as the kanji, "雪"(Snow), with the difference in the vocal readings. Based on what I've watched, the protagonist's name "Setsu" was carried over to the other world when he got isekaied the first time. After he defeated the demon lord Toma and transported, he got the name "Yuki" (and presumably reincarnated as child) in another world/timeline on the modern days, before he got isekaied the second time.
