Update to Tier 4 (Patreon)

I have updated the description of tier 4 members (See Above)

The main differences are:

* Reminder to reach out to me for every month that you are a tier 4 member with your request

* Videos for tier 4  can go on either YouTube or Patreon depending on the best fit for performance.
