The first guy who got smallpox, Andy, was a kind of casino shill who arrived at the Bella Union as if he were a big money gambler and the Union was a high quality establishment that was as legitimate as the city gambling joints back east. He came into the Union and Cy pretended that he didn't know him to the regular customers as part of the con. Of course, Cy and Joanie all know who he is as I presume he's done work like this for them for years. He wasn't in the background or anything like that so you didn't miss him from the first couple of episodes. No worries. He was just introduced in a really grandiose way as part of the game Cy is playing with his customers to drive up interest in craps.
