Izumi wanted nothing more than a child. And the Elrics wanted a (their) mother. In a way, they each got their wish.

*not spoilers as none of this is every really explained more later* for some reason the english version refuses to say it properly but Izumi didn't lose "part of her internal organs". specifically in the manga it is as a result of trying to bring back her stillborn child that the Truth took her womb. making it impossible for her to attempt to have children again. and it was ripped out haphazardly like when Ed lost his arm or leg, which is why she vomits blood. The episodes also fail to make clear like in the manga why Izumi and Ed can use transmutation without a circle. alchemy is performed using a circle. the circle is the boundary of the work being performed and what is contained within is what work is being executed. thing of it like code within a program. everything within is the rules, and the circle is the beginning and the end. and learning to perform alchemy is like learning how to fill in the code. Ed forms a circle with his arms and within his mind using the knowledge gained from the Truth, he fills in the rest and transmutes. all things beginning and ending in his hands and filled in by his heart with what was given to him by God/Truth.

also fun fact about Izumi that got cut from the show entirely. the reason why she as an alchemist taught the boys how to fight is a result of a mistake on her part when she was young and wanting to learn alchemy. she heard of a pair of brothers that were good at alchemy and wanted to learn from them. she got the names wrong and wound up asking to be taught by different brothers with a similar name. they were martial arts fanatics. when they accepted her they dropped her off in a frozen tundra in the north for a month as her initiation where she survived by hunting the local animals possibly up to bears and raiding the local military garrison for supplies when she could. after her month she discovered her mistake and went to find a proper alchemy teacher but still kept the martial arts training and the 1 month initiation arc she later used on the boys on a thriving island.
