I personally think that Lorne never read Knox. Early in the season Fred tells Knox he doesn't have to go to Lorne to be read, that she trusts him. I think maybe Lorne assumed he did a reading because he was supposed to read all the W&H employees but I really don't think he did - and he shouldn't blame himself for "missing" a sign.
