So the whole clan thing is a common trope in anime, idk if you guys know this by this point. But clans aren't the same as families. Clans specifically are collectives based around a common male bloodline. It comes from the feudal era where a feudal lord would take power and build up the male bloodline as strong as possible, so when he dies, rather than a new feudal lord taking over, the men of the clan would seat the eldest clan member and fight off the attacking force. As a result, clans would typically marry off women either as a negotiation tactic or to bring strong blood into the clan like horse breeding basically. So in JJK it seems like clans are a sort of structure for passing cursed techniques through male bloodlines in a similar way, so all of Gojo's distant relatives aren't part of his clan because they're not part of the 6 eyes an limitless male bloodline - likely they're related from a female clan member being married off to the zen'in clan or something to that effect
