We knew Norma had a brother before this episode. At the end of season 1, Norma confessed to Norman that she had an older brother who constantly made her have sex with him when she was a teenager and it went on until he moved out years later. That's also how she got the scar on her leg, which the psychologist pointed out when her and Norman went to see him.
