Sakuna attack is slashing. His domain expansion basically slashes everything until nothing is left. Cleave is slashing anything with cursed energy (people, living things, cursed tools). Dismantle slashes anything inanimate (building, phones). He just cuts stuff. There’s no barrier like other domains, ie Megumi broke into Dagons domain and Yuji into Mahitos. Since his has no barrier it means people can enter or leave will so it compensates by having a larger range.

To explain the ritual: Ten Shadows users are only given 2 demon dogs to start. To get all of the other shikigami the user has to start a ritual, and beat the shikigami into submission to be able to control them. So megumi has summoned, fought, and beat all of the shikigami he uses. You can also have other people join the ritual to fight the shikigami BUT if there are other people participating in the ritual, then you cannot control the shikigami afterwards you would just have to summon it again and beat it solo. So megumi forced haruta to participate in the ritual as a “I’m gonna die and I’m taking you with me” move. But megumi was hit and sent into a near death suspended animation, because until the ritual is complete (the shikigami is dead or all people participating in the ritual are dead) he can’t fully die. So sakuna came in, healed megumi, and then killed the shikigami.

Anime-only watcher here, so I don’t know what Sukuna’s relationship is with Megumi, but Sukuna purposefully limited the radius of his domain hits to 140 meters so Megumi wouldn’t get hurt, and limited the attack to only things on the surface, hence why Nanami survived. Can’t wait to see your next reaction.

Sukunas Domain expansion has a max range of 200 meters in all directions. Since it is not an enclosed space like all others, the compensation is a wider range but that means anyone can run away from it "in theory" as they are not trapped inside of it. But Sukuna limited the range to only 140 meters as to save Megumi from it, as he was just beyond 140 meters. Which means that everyone who was not within 140 meters survived. And so everyone you saw Nanami included was further away then 140 meters. Simple And I don't know if you picked up on it, but the flashback showed just how strong that shikigami is as he stated that one of Saturos predecesor who also had both of the gifts from the Gojo clan, so basically a character as strong as Saturo and a top tier character from the Zenin clan both died to that shikigami when the Zenin character summoned it.

Seeing the Shibuya arc animated has been amazing, and the sound track, like you mentioned, has been awesome this season. You clapping and saying ‘please’ then shouting ‘daddy!’ at the end was hilarious! 😂 Thanks for another fun reaction!
