Most of time when they talk about equivalent exchange, or breaking it, they are talking in a literal sense but they do sprinkle in figuratives (or maybe it would be better explained as unknown to them) as well (normally souls). Alchemy advancements in their world only allow them to mess with things on the molecular level (as in they can break H2O into just hydrogen and oxygen and use them separately or recombine them with something else but they can't change oxygen into hydrogen) but a philosopher's stone allows you to brute force that by trading what the stones made of (you will find out later) for temporary access to that knowledge (like how Edward traded part of his body and soul, accidently, for permanent knowledge on how to make a transmutation circle without drawing it). This allows people to change "blank" molecule into a different one. It is unlikely, though, that they can make matter, they just change oxygen, or whatever is abundant, into what they need.
