Like adoptdon'tshop said, dean's nightmares were telling of the three things that are really weighing him down: his time in hell, losing Cas, and having killed Amy. John raised dean to feel responsible for everything from a very young age, he was responsible for the safety of another human being (sam) since he was 4 years old, that kind of prolonged pressure & stress leaves a lifelong affect on u that is extremely hard to break from, he wasn't just responsable for sam's protection but also having food on the table & basically being sam's mom, dad & big brother & if anything bad happened to sam he had to face john's wrath, of course he will grow up to think that everything is his fault/responsibility & that his life isn't important.

I also love that u mentioned that most of dean's actions are drived from a place of not wanting to be alone, that is very true, dean has this belief that everyone leaves him & that he's poisonous & that's his biggest fear, that fear is always there & it leads him to make decisions he wouldn't make if he was a more secure & confident person, dean's biggest villain has always been john & almost all of dean's issues can be traced back to him 😢
