So about cyber psychosis without spoilers. In the TTRPG it is a state that prevents players from over chroming. Basically each chrome part take away from your Humanity stat and without that you're just turn into a killing machine. In 2077 CDPR expanded on this by telling that cyber psychosis is more so just a mental state that caused by excessive, malfunctioning or not properly installed augmentation. Following your example with extra ear, it might start triggering not only hearing neurons in your brain but also pain or taste. And it might drive you mad in a long run. With clean chrom, tailored for specific user and proper software it shouldn't be an issue, though not everyone can afford that. In the show they don't dwell deep how it works but you can assume that David has naturally high Humanity stat, that he can endure not well balanced chrome for pretty long time already.

Also not all cyber psychosis is violent or result in shaky hands, it's just how this show conveying that feeling.
