Alicent and Otto know Aegon isn't fit to be King, but I have a feeling they believe they'll be able to rule through Aegon and just use him as a figurehead. Cersei thought she could do the same with Joffrey but like Tyrion told her, it's hard to tame a dog once you put a crown on its head.
