I was thinking that Derek came by to chew Stiles out for not coming to a pack meeting only to discover him like this. Which begs the question - how long was he like this before Derek came by.

I was really feeling a rune vibe for this - like the person tied him up and tried to get answers from him - beat him quite severely before realising they were getting nowhere and dipping a finger in stiles' blood and drawing on a healing rune and loosed the ropes and left.

Stiles' magic with the rune would make him heal faster

I also feel liek they were searching his body for some kind of mark which is why his clothes are in disarray

Stiles perhaps has learned enough about this strange symbol that manifested to know to hide it so he was just lucky that the person came pre shower.
