The ones he couldn't save were the children that were living in the cave with him when the church came and murdered them while he was out.

It was the kids in the cave from the first episode, when it showed them killed and impaled and set on fire, saying they were saving them from being corrupted by the necromancer. He reacted in kind by torturing them for a hundred years without the release of death

In a nutshell yes. Well not really in hiding, he just didn't want anything to do with people anymore until the kids showed up, but yeah in that order

For the kid Lei assassin it was already revealed he had lost all his limbs. Next episode will be nasty, will be interesting to see what spot they will leave people hanging. Btw Polka's past is still too disjointed for you to get the clear picture hopefully people won't spoil too much as you will get clearer picture how his life went there later.
