I think it would have been better to differentiate the poll a bit. For example with a fourth option like "mild cbt is ok". I think a lot of people think of ball kicking or harder stuff when they hear cbt and are turned off. But I think most people wouldn't object to lighter stuff. Two examples what I mean by that: For example an extended scene where Alex fucks the player with a bigger dildo/strap-on (because his little hole got accustomed to the old one ;)). If the player refuses to open his mouth to receive his cumshot Alex crushes/squezzes his balls which results in the player opening his mouth and an extra strong cumshot. Another scene I have in mind would go as follows: After increasing the players cock Alex would viciously mention that now that the player has decided to have a bigger dick it's time to get some corresponding bigger balls and enhance his little sack a bit. The player would temporary wonder what's she means by that only to be surprised by Alex with a ball-stretcher (or some saline infusion, but not sure about that). While she takes him on a walk on all fours with his balls prominently on display they could meet different people (maybe a repeatable scene with different encounters) who would comment on the player characters ordeal and maybe humiliate him a little bit. Alex would encourage the player character to move by occasionally lightly beating his balls with a riding crop. This scene could also be a starting point for an extended scene where Alex gives the player character a break and shows him some affection. For example an asshole customer could at first touch or finger the player character with permission of Alex only to really try to hurt him and crush his balls. Alex would quickly dispatch the asshole and check if the player characters testicles are still ok by giving him a handjob/blowjob as compensation/reward.
