A chunk of this episode was a tribute / homage to the film Rambo: First Blood. Or so it seems to me.

Yeah, she probably hasn't seen FB but yeah, it was definitely their way of paying tribute to it. Them being in the woods after escaping prison, the soldiers hunting them down but they become the hunted, etc. Plus of course the title.

Seems that the point of this episode was to show how important the boys - all Winchesters - are to the world. There are other hunters out there, but none like them. Other hunters call them for help. This episode could have done a better job of showing that, I think. About Billy - I can only assume that billy was happy to do anything that would get her a dead winchester. She made a deal so had to stick with it. It did seem pretty loose, but a decent episode. Nice seeing the boys showing their prowess against other humans. About time! lol

The episode title is because this episode was an homage to the first Rambo movie also titled First Blood

"we're not trapped out here with you. You're trapped out here with us" ... is one of my favorite lines in all the Supernatural episodes! And watching this episode made me want to re-watch Rambo First Blood again...so I did!
