Enzo's writing is a bit all over the place, in large to him being - as you pointed out - reduced to a mere plot device in season 6. It's kinda best to just approach Enzo every season as if he was a new character :D

Enzo wanted to kill Stefan for, in his mind, abandoning Damon in pursuit of his own happiness. So Enzo decided he was going to fulfill Damon's promise of a lifetime of misery. It also didn't help that Enzo was kind of crushing on Caroline at the time and Stefan kind of broke her heart so he was avenging her, too.

Also, they did foreshadow the orphan stuff when Lily found him. That's why he was so attached to her.

I would bet that Caroline couldn't breast feed. As she pointed out, one of the perks of a vampire pregnancy was that her body healed right away after she had the babies. I'd bet that means her body is just like she'd never given birth at all, so she probably doesn't produce any milk.
