So for the show name, the Rail Zeppelin part comes from the arc it adapts in the 2nd half, and the Grace Notes references to the first half, basically the anime skipped the first three books/ first 2 arcs because they did not think it would make for a good show as it was very internal monologue heavy with 0 action and replaced it with Grace Notes which is canon but not in the original books as a way to introduce the cast of the show.

So the part where waver was upside down was 7 years from the present of the show, (it had a quick flash saying 7 years ago) and three years after Fate/Zero. The present of the show about half a year before UBW. Reines is 8 years old in the upside down scene so yes a kid, she is 15 in present time So there are lots of different mystic eyes that do all sorts of different things. Different mystic eyes have different rarities with Shiki’s mystic eyes of death perception being the rarest of them all. UBW Rider has mystic eyes of petrification that turns people to stone as another example. That dude had Mystic eyes of Binding so he can bind people in place when he looks at them. Reines’ mystic eyes don't have a name to my knowledge but they let her use her magcraft very precisely, more so than the average magus. Reines does not have her own anime, but she is a major supporting character in this show
