I think the 2 demons in the Mugen Train wasn't real; it was part of Rengoku's dream since he was already under Lower Moon 1's spell at that moment. Tanjiro wasn't born with a scar but he got it from protecting others. He might not be born as one of "the chosen ones" but his actions and efforts made him worthy of a chosen one, which is basically the main theme of this series. Tanjiro wasn't breathing when he went into berserk mode, so his sibling came to remind him to breathe or else he would've died.
