That's clapping you hear with the water falling. Geto is hearing the clapping from the end of the last episode. That moment had an effect on him.

The star religious cult people clapping, the water from the shower scene, the rain pouring outside, all happen when the dark thoughts are coming through. I didnt notice that until re watching here. That is pretty damn good writing and direction considering it must have been adapted from a manga with no audio. The problem of only a portion of the worlds population creating a threat to the entirety of them all is interesting and similar to AOT. Eliminate the source of curses (titans) by killing the non-sorcerers (eldians) Zeke would be proud.
