結論としては、Patreonのスマホアプリ版からはDiscord連携をする設定が出来ないのでWEBブラウザからやってくれとのことでした。

なので、スマホユーザーの人はWEBブラウザでPatreonのページを開いてログインをして、そこからはPC版と同じような感じでDiscord連携が出来るようです。
