Would you fill up a short Formulary after your order is complete? (Patreon)

Hi there guys :3 

I would like to create a short Form to be sent to every client after their order is completely finished :3 it would be to answer things like if the final result had the expected quality, if was worth it the price, if the process was professional, if to contact me was easy/fast, if the time it took was reasonable according with the complexity of your order, few questions like these :3

The last question would be an optional Testimonial which I would add it in my website :3 (my website is still under construction, but is finally starting to be a thing!)
So... do you like the idea?
