The sub titles are different because they are for the Japanese voices, while the English dub is translated a bit differently because of the mouth movements mostly, so they can't be the same.

Is Alicia ok? I felt a sudden, inexplicable feeling of sadness when I was at my training just now. Has something happened? I don’t usually believe in stuff like that but I know she hasn’t been feeling very good after her surgery and I have no idea what caused the feeling.

Turns out one of the idiot surgeons left the implantation equipment in her and fucked up her organs. Thankfully it wasn't fatal, but it's another setback in her getting treated. (And a potential lawsuit for that act of medical clownery.) So she's obviously not feeling good right now.

The subtitles arent 'bad', they're accurate to the original japanese version of the anime, it's actually the eng dub that's inaccurate if anything. (And no, I have no clue why they used the same translated subtitles for eng and japanese)

It came out of my body one night which caused me to decline. Today I went to my surgeon and she took out the implant itself. Everything went wrong but in a few weeks I should be a lot better!

Man, I watched this in sub, but man whoever voices Legoshi has done a fantastic job. I've already seen the opening part before but his use of words and how he said it still made me feel just as uneasy.

That's common in anime, the dub often changes lines to match the lips of the characters usually.

Jonah Scott voices legoshi, he does a few other roles in anime english dubs and is voicing a character in an independent youtube series (similar to Helluva boss, infact it's made by someone who works on Helluva boss) called Far-fetch'd and he's incredibly social online and it's so awesome to see. He's done a lot of videos talking about his roles on youtube and he even stars in peoples videos (like uh... one infamous clip where he confirmed Legoshi has a 12 inch cock...)
