Due to facts that have yet to be mentioned in the story, I believe there is an argument that the inhabitants of Peachy Island consider themselves to be both AND neither a man or woman. I could be wrong though.

New Kama Island, and by extension the people associated with it, is related to the concept of "Okama" which is essentially the Japanese word for queer. More specifically, it includes gay men, trans women, and straight men who are feminine, crossdress, or exhibit stereotypical traits associated with being gay, like a high pitched and/or overly expressive voice. Essentially, New Kama Island is a place for men and trans women who didn't fit into the regular One Piece societies. But yes, some of them, like a certain two big names, would be considered genderfluid, male and female. And though not confirmed, some are certainly nonbinary or otherwise nonconforming.
