Pretty sure Clara's blue button-down shirt was what she was wearing under her space suit. Before she gets the leather vest from the Vikings, there are scenes with her wearing the blue shirt without it (see: the Doctor translating the baby scene, for example). Nothing weird.

Also, we don't know for sure, but I'm pretty sure that when Ten wore the spacesuit (and maybe the same goes for Eleven and Twelve), they wore their usual 'Doctor clothes' underneath. Like in '42', when Ten asks for the spacesuit to try and save Martha from falling into the sun, I'm sure he didn't strip down to his underclothes to put the suit on, but instead, just put it on over his clothes. So, again, not weird? 🤷‍♀
