The creators have talked about how they liked trolling people with the fake intro. There were actually a few hints in the first season of the show. The first big hint is during the testicular torsion episode. When Dr. Venture and Billy are talking Dr. Venture mentions something about he could have fixed or prevented this in the prototype phase. Also, if I remember correctly the song at the beginning of this episode was something they had to fight to get because of the cost. The creators are big music buffs, you'll see tons of music references in this show. Also the multiple death scenes is a classic scene that never gets old.
