Those were addictive substances and even then he warned that the kids would be in for a long hard recovery process, much like getting over real chemical addiction. The smile effects are closer to df drawbacks, so to cure them would mean to cure a person of a df. The only cure I know of for devil fruits is death.

So the lower number of guards inside makes sense if you consider that the only entrance is guarded by a virtual fortress. Of course that fortress's security was compromised by the guards being drunk, but in all fairness they were likely informed by Orochi to not expect any troublemakers because all of the ships, including the Sunny were (supposedly) destroyed. They probably didn't believe that such a fortification could be destroyed so quickly and completely without anyone alerting someone within, so the purpose of the smaller guard station within was likely to screen the incoming guests for impostors and stowaways.
