Not sure if you got the joke in the beginning of the episode, but just in case you didn't, Simon's joke about growing a black mustache is a reference to villains in Victorian-era plays, and later on into silent films and '60s cartoons, always having black mustaches. Firefly aired from 2002-03, not the '90s. Interestingly enough, the first regular non-heterosexual character on an American television show was in The Corner Bar which aired from 1972-73. Niska is played by Michael Fairman who's mostly known for his work in soap operas such as Ryan's Hope. Detached limbs should be kept cold, but never directly in contact with ice. Ice could cause freezer burn to the blood vessels which makes reattachment difficult, if not outright impossible. Book didn't really "dodge" the bullet Matrix-style like you think. Really he just saw someone firing at him when he was very vulnerable and tried to lean his head towards whatever cover was nearby. Nothing supernatural at all.
