ALL $10 & $20 PATRONS, FILL OUT THIS FORM! (Patreon)

As discussed before, all $10 and $20 Patrons will have their own personalized icon made to be shown at the end of every Diamond Axe Studios Music main-series video.

I have created a form for you to fill out, detailing what you want your icon to be. This is a NEW official form that we're going to be using, so even if you've already filled out a form describing your icon before, PLEASE DO IT AGAIN on this new form (unless your icon has already been completed as listed below).

Naturally, it will take a while for all of the artwork to be completed, so in the meantime, a generic placeholder image will appear with your name in the videos.

Furthermore, if at any point in the future you change your Patron name, please fill out this form again with the relevant information. If you do not do this, we will not know that you changed your name, and your icon may be removed by mistake.
