That first page will be quite an interesting art challenge. You see, normally when I do inside views, I set the light around where the camera is, since normally there would be no light at all you know. But here the author actually specified how the viewer(and the character) will be able to see inside the person. For the mouth there will be light shining through the cheeks, as they thin and would let light shine through(similar to how you can still tell bright light sources when closing your eyelids). The concept sketch showed it a tiny bit on how i envisioned the shadows to work.

Later on in the story when he is the stomach, there will be a different method used though. But let's ignore that for now.
