You're pretty much right about when sometimes the dialogue doesn't match the video. It's called overdubbing, and is commonly used when they bring in an actor to re-record some of their dialogue because the video had some problem (like a plane suddenly being heard in the background of a shot.) But, yeah, always been and continues to be very common.
