Here is the Fantasy Form to pick your captain for episode 5

Remember for picking your team captain you have to choose someone you haven't already chosen as a captain picked (rule is for the first 6 eps). As well as whoever your captain pick is can't be someone you chose as one of your team members
Only use the bottom of the sheet to pick a replacement if you had either Luis or Amanda on your team
