Harlow was inspired by the condition known as microcephaly. Made most famous in the modern age by Tod Browning's Freaks and more recently American horror story.

It is a rare neurological disorder and is characterized by a smaller than average head. Biologically, during conception the head fails to grow in time with the face – which continues to develop at a normal rate; this produces a person with a small head and a receding forehead.

While usually this does impede on mental and physical development there are also instances of it having little effect at all. The most famous individual being William Henry Johnson aka Zip the Pinhead, who not only endured the carnival circuit but became one of the most wealthy and celebrated acts of his time. Owning multiple properties and is still remembered today as a famous icon.
