I'm thinking Lilith was the patient zero here. Her plan was very simple, make the Exterminators(as Adam is the sole guy from what we see) into Bimbos too busy screwing to doing any exterminating, which would distract Adam for obvious reasons. She needed to be in Heaven for it, but once there she just let her own bimboness seep into the environment, slowly Bimboing every woman in Heaven. Sera's been fighting it, but is on the verge of giving in, hence her rivaling Lilith's size. Emily didn't realize there was anything to fight, and went Bimbo pretty fast. Since Heaven is no longer counterbalancing Lilith, the women of Hell have also been swelling up, Hence Vaggie getting bigger, based on her picture, and Lilith might have gone up a bit in size as well due to 'cooking in it's own juices' as it were, since the women of Heaven have ended up as Bimbos.

The succubi changed fastest, and since they idolized Lilith, they've all been hoping to get ultra-busty like her, which has lead the 'bimbo essence' they absorb to mostly go to their breasts.

Additionally, Sinners and Winners have been regaining their fertility once they go full Bimbo. I'd say Carmilla is, like Sera, about 90% Bimboed up, mostly do to the Exterminator gear she and her daughters handle all the time, and is well aware the three of them are an inopportune boner away from becoming complete bimbos, though since all the women are changing she hasn't figured out where it's coming from. She's hanging on mostly through sheer willpower, but she's at her limit, like Sera.

Not quite sure what you mean by 'Since heaven is no longer counterbalancing Lilith'. Sounds like that if Lilith simply stayed in Hell both sides would have been fine? I'm not sure how her being in Heaven is causing women in Hell to swell up as well, because if that's the way it worked, she could have just stayed in Hell to begin with? I guess this also implies that Lilith was turned into a bimbo somehow, that she wasn't just one to begin with.

Basically, her True Form was at least 0.5(when she entered Heaven she would have swelled up about 4 cup sizes the second Adam was gone), with Heaven's influence curtailing any further change/spread. Once she was actually in Heaven, however, she was 'past the firewall' as it were, and while it took a couple years to build up a head of steam, as of year 4, most women were visibly larger from month to month. With Heaven's women 90% Bimbo, Hell didn't take long, though Exterminator gear remains the main vector, but once a woman changes enough, she becomes contagious herself, to the point Carmilla and her daughters have unwittingly been pushing each other more into Bimbohood just based on proximity. IE, wanting to have assets more like their mother pumping them up, and fantasies about having more daughters widening Carmilla's hips, respectively. Emily went Bimbo as of year 3, and was one of the earliest full Bimbos. This is probably the point when Heaven's women started going bimbo en masse, though most of them didn't change all the way until year 5. At that point, Heaven was working for Lilith rather than against her, and Hell's already a den of sin, so it didn't take very long, despite being so far away. Lilith's just waiting until the Exterminators are more concerned with sex than Extermination, or until Sera succumbs to her Bimbo side(she's already having more and more moments of vapid lust) so Lilith can just fuck her into submission to get her to call the Extermination off, to little fanfare with the Exterminators so thoroughly Bimboed. Lute's actually one of the less affected, being so focused on Adam that as long as he's for the Extermination, she'll be almost impossible to sway.

Alternative hypothesis: This was all a very convoluted scheme to get Lucifer(and herself) Sera and Carmilla as 10,000 year anniversary presents. Lilith knows that Lucifer prefers tall big-assed women, and she knows Carmilla's going to get huge no matter what(and Carmilla's right on the edge, hence her looking at her swelling ass in disdain for making her a slut waiting to happen), and her daughters(including Velvette if you believe that theory) can then be foisted onto Charlie, who, if she's taken in a quarter of the lessons Lilith tried to teach her about pleasing a woman, is a sex demigod that Vaggie is struggling to keep up with by herself. Fucking Sera serves the dual-purpose of reversing Heaven and Hell's positions by literally making Heaven's head honcho the ruling couple of Hell's bitch. Plus she'll be out of the way of any schemes to include Emily in Charlie's harem. Or it could be both.
