The dolls screamed like banshees because they were being filled with souls from horrible deaths. When Hohenheim unknowingly absorbed souls from Xerxes' citizens, those souls were in grief too. Hohenheim could hear their cries. He felt guilty that he had a part in it but couldn't do anything about it, not even die. I bet he's been depressed for a while.
