I think the surprising idea is that alchemy is a thing that Ed can sacrifice at all. With modern medicine I can give my kidney to my brother, but wind the clock back a few hundred years and that isn't even something I can imagine. This is the paradigm shift that Ed brings to the table in the last episode: alchemy isn't just revealed truth, it's a connection to Truth: a transaction that can be renegotiated.
