There is a popular fan theory that "fell down the stairs" is a phrase used to describe someone that has committed suicide. So Kuina could have taken her own life. However, this isn't confirmed. But I love this backstory because either way it show Zoro's ambition and the importance of life and how quickly someone can be taken from you. Great episode.

I'm pretty sure the suicide thing is indeed entirely a fan theory; like, some people suggest it's a known euphemism in Japanese, but that part isn't true. Still possible, just no reason to think suicide based solely on canon. 100% agree with the comments on Zoro's motivation and fragility of life. In fact, I would say those themes are stronger if it really was nothing more than a random purposeless mundane accident.

this whole episode with Kuina and how her father instills in Zoro a sense that women are weaker/fragile, which is reinforced by her death, really seems to be the root of Zoro's major character flaws in the show. I am really interested to see if Oda handles the opportunities for Zoro's growth well, or if Oda is unaware of the inherent misogyny he has created in how Zoro acts in the show (arguably even more than another character who gets misconstrued as a misogynist bc he has "downbad for all girls" style jokes)
