Because he doesn't care what the nurse thinks about him. He cares what izzy will think when she finds out. It was tied into the idea of the episode, it's a mental issue.

I think the two of them felt it was easy and there was no problems. Unlike their other partners who they have nothing but problems with. Which I think it can be very liberating to have something that is just no complications (as long as you don't get caught lol 😂)

With the nurse it's just sex, no feelings with izzy it's all feelings with a side of sex and he wants to be the type of person that she can be proud to be with. He doesn't feel like that guy when he feels like he is "failing"

The look on Burke's face when he sees how messy Cristina's apartment is... hilarious. With the Meredith and her one-night stand mishap... I've always felt Derek was a bit snotty about it. Like, I understand he wasn't expecting to feel so bad when she started dating again, but he was a bit judgey. And given that he never told her he was married, I'd say he has NO right to judge. Plus, he made his choice, so he gets no say in Meredith's personal life anymore.
